Making Your First Appointment
Call Ultrahealth at 415-986-4979 to schedule an appointment. We will do all we can to find a date and time that work for you. You'll be asked for contact, referral/injury and insurance information. If you have health insurance, please have your card available. After you call us, we will contact your insurance company to obtain coverage and eligibility information, including your co-pay. If there are any problems, we will notify you immediately.
It is not necessary to use health insurance - some people don't have health insurance, and others just choose not to use it - nor do we require a physician referral (this requirement is determined by your health plan). Let the receptionist know if you are planning to pay out-of-pocket.
You can fill-in and download new patient forms from this website, or you can choose to fill out these forms when you arrive. If you have not completed the forms prior to your first visit, please arrive 10 minutes early.

What to Expect Your First Visit
Your first visit will last approximately one hour. During that time your physical therapist will evaluate your injury and then formulate clinical goals and a treatment plan consistent with your own goals and expectations.
Bring comfortable clothing - we recommend shorts, T-shirt (sleeveless if you can for shoulder problems) and athletic (e.g. running) shoes. You'll be asked to change in our locker room prior to seeing your therapist. We suggest arriving a little early to allow time to change and fill out paperwork (if you haven't already).

What to Expect After Your First Visit
Subsequent visits last 30 minutes to an hour and consist primarily of delivery of care - exercise, manual therapy, and other therapies such as electrical-stimulation and ultrasound. Education and re-evaluation are provided as required. As you progress you might notice a shift in emphasis to exercise. At some point your therapist will give you a home program. Depending on your treatment plan and the progress you are making, you may spend a portion of your time at Ultrahealth with a physical therapy aide, typically working on your exercises. This allows us to provide extended care as well as better monitor your progress - we don't have to rely as much on your home program. You may also be asked to attend back class or circuit class.
You are strongly urged to stay on plan, both in terms of frequency (number of visits per week) and duration (total number of visits). Your treatment plan is formulated to maximize the speed and effectiveness of your recovery. Scheduling ahead is always a good idea - it allows more flexibility in picking dates and times that work for you. If your PT is ill or on vacation, it is better to schedule with a replacement PT than to skip a visit and get off-plan.
If you are ill, out-of-town, in a meeting, or have to miss an appointment for any other reason, please contact us at least 24 hours prior to your visit. Try to reschedule missed visits so that you stay on plan. If you anticipate missing three or more visits, please let your therapist know - they may need to revise your treatment plan.

What to Expect After Discharge
Typically you will be discharged with a home program. We will encourage you to keep doing your exercises after discharge, for as long as they are beneficial. We will also encourage you to resume your active lifestyle or perhaps become more active: We might recommend one of our in-house fitness programs - circuit class, personal training or gym use - or we might recommend a community resource.
After you are discharged, please do not hesitate to contact your therapist for any reason - assistance with your home program, how to cope with a flare-up, or just to let us know how you are doing. Our goal is to see you fulfill your own goals - whether that involves just walking around the block or completing your first marathon - so we always want to hear from you. If necessary, your therapist will schedule additional care.
